Indeed you were right; I just reseated the card and cleaned up some of 
the PCI connectors while I had the card out, and it's now registering 
the correct IDs and being picked up by the driver. I hadn't thought this 
might cause the problem since it was working enough to be picked up by 
lspci and showed the same thing after multiple insertions.
